About Us
REM MN Community Services, Inc. offers a wide selection of services and programs for individuals living with disability in Ramsey, Anoka and Washington counties. Service options include Community Supportive Living homes (24/7 care), ICF/MR sites, In Home Hourly Supports, In Home Parenting supports, and Family Foster care services. Two programs (Hemingway and Hearthside), located in Cottage Grove, MN, follow a Housing with Services model. Individuals served by REM MN Community Services, Inc. live with brain injury, mental health issues, intellectual developmental disability (including autism spectrum disorder), dementia’s, complex medical needs and other physical disabilities. Funding for services generally comes through medical assistance, county waivers (CADI, MR/RC, EW, TBI NF, TBI NB), workers compensation, private pay and other insurance.
